Through the Street Outreach program, Finding Hope works with those struggling to help them find stable housing, services, supplies and food. We focus on helping them to develop relationships that empower them to rebuild connections and rise out of poverty. 

Street Outreach services include: 

Street-based education and outreach – Providing information and helping people access programs such as Veteran services, SNAP, Medicaid, housing applications, etc.; assisting them in obtaining vital documents (driver’s license, birth certificates, Social Security cards) so they can gain employment, disability benefits, or apply for housing, free phones, Medicaid, SNAP, and so forth.

• Guide to community resources
• Survival aid - We provide food, needed first aid (bug spray, sunscreen, Neosporin, etc.) tents, blankets, cots, clean clothes, socks, shoes, hygiene items, etc. We also assist with connecting to a PCP, mental health services, and arranging transportation to medical appointments.
• Household furniture and supplies (case by case)
• Emergency clothing, milk, diapers
• Emergency food boxes
• Referral to treatment and counseling
• Follow-up support
• Emergency housing –  We provide tents and sleeping bags to those who are reluctant to go to a shelter or have pets they do not want to leave. We refer many to the out of county shelters. For those who may need other long-term services, assisted living and nursing homes, we help connect them to the appropriate provider and assist with paperwork and acquiring vital records when needed.
